The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* deleting from item 7b the zip code of “01747” and adding “01746”
* deleting from item 12f the entry “0000-05-22” and adding “0000-06-09”

THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT’S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) as follows:

* item 7b (Home of Record (HOR)) to show the zip code as 01746 vice 01747
* item 18 (Remarks) to show his deployment dates to Afghanistan as 20140214-20140822 vice 20140301-20140822

2. The applicant states the information is incorrect.

3. The applicant provides:

* DD Form 214
* Deployment/Redeployment Verification Memorandum
* Temporary Change of Station (TCS) orders

CONSIDERATION OF EVIDENCE:

1. The applicant enlisted in the U.S. Army Reserve under the Delayed Entry Program (DEP) at the Chicopee, MA, Military Entrance Processing Station on 15November 2010. Item 3 (HOR) of his DD Form 4/1 (Enlistment/Reenlistment Document) shows his HOR as “Holliston, MA 01746-1737” and place of enlistment as “Chicopee, MA.”

2. He was discharged from the DEP on 23 October 2011 and enlisted in the Regular Army (RA) on 24 October 2011. His Enlisted Record Brief, which was created upon his entry on active duty, listed his:

* HOR as “Holliston, MA”
* deployment dates for his current combat tour as 20140301 to 20140822

3. He was honorably released from active duty on 16 April 2015. His DD Form 214 shows in:

* Item 7b “xxxxxx, Holliston, MA 01747-0000”
* Item 12f (Foreign Service) ) 0000-05-22
* Item 18 “Service in Afghanistan 20140301-20140822”

4. He provides:

a. TCS Orders CR-361-0041, issued by Headquarters, 4th Infantry Division, Fort Carson, CO, on 27 December 2013, ordering his deployment to Afghanistan in support of Operation Enduring Freedom on or about 1 February 2014.

b. Deployment/Redeployment Verification memorandum, issued by his platoon sergeant on 28 April 2015 and confirming he deployed to Afghanistan on 14 February 2014 and returned on 22 August 2014.

REFERENCES:

Army Regulation 635-8 (Separation Processing and Documents) prescribes policy and procedural guidance relating to transition management and explains separation document preparation. It states the DD Form 214 is prepared for all personnel at the time of their retirement, discharge, or release from active duty.

a. Item 7b shows the Home of Record. A Soldiers initial enlistment contract or appointment document is the source for this data, or any correction approved by the U.S. Army Human Resources Command. List the street address, city, state, and zip code listed as the Soldiers home of record.

b. Item 18 shows mandatory entries and entries too long for their respective blocks. One of these entries, for active duty Soldiers deployed with their unit during their continuous period of active service, enter “SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates)” duty.

DISCUSSION:

1. When the applicant enlisted in the DEP/RA in 2010, he listed his HOR as the city of Holliston, the State of MA, and the zip code of 01746. When his DD Form 214 was processed, it erroneously listed the zip code as 01747.

2. The applicant deployed to Afghanistan from 14 February 2014 to 22 August 2014, a period of 6 months and 9 days. His foreign service is incorrectly captured in item 12f and his deployment dates are incorrectly listed in item 18.
